Data collected is utilized in our online memorial, slideshows presented Memorial & Veterans Day weekends on Berlin’s Cable TV Channel. DVD’s & CD’s of the show are available for checkout at the Berlin Public Library or can be purchased by  contacting the foundation.

Veteran Eligibility

Berlin area service men and women from Civil War veterans to present service members, who have honorably served our country in every way. Be it in peacetime or war, including all branches of the service such as Army, Navy, Marines, Air Force, Coast Guard, Merchant Marines and their affiliated.

Data Collection

The committee needs your help in locating and documenting all veterans of the Berlin area!

What information does the foundation need?

(1) Proof of honorable service.  If you do not have proof of honorable service, please contact your local Veterans Service Officer:

    * GREEN LAKE COUNTY: (920) 294-4123
    * WINNEBAGO COUNTY: (920) 729-4820
    * WAUSHARA COUNTY: (920) 787-0446

      If your county isn’t listed, please check this website > http://dva.state.wi.us/CVSO.asp.  On the left of the screen there is a list of other sites. Click on Veterans Service Offices. A map of the state will appear along with an alphabet. Click on a letter and the counties starting with that letter will appear and so will their contact information.

(2) A picture of the veteran in uniform and a most recent photo.

    * We can take a photo of the veteran when info is brought in.
    * Or we can crop the veteran out of any group picture of your choice.
    * NOTE:  Pictures will be returned upon request.

(3) Data Collection Checklist

    * Please fill out the Data Collection Checklist
    * If you have more info on the veteran or if you don’t have enough room on this form, we encourage you to send us any additional information. All veterans played an important part in history.  Their information is valuable for educational purposes as well as for historical documentation and for genealogy research by future generations.
    * Please mail all supporting documentation to the address listed below.
